If an NHS Continuing Healthcare assessment is on the cards, the NHS Integrated Care Board (ICB) will make contact with you to arrange a suitable time and date. These meetings can take place in person or virtually online. During the assessment, a Multidisciplinary Team (MDT), usually comprising a Nurse and Social Worker, will complete the Decision Support Tool (DST). They will then make a recommendation on eligibility for NHS Continuing Healthcare Funding.
