When an employee notices incorrect processes at work that could be dangerous or discriminatory then they have a duty to report this up the chain of command. This doesn't always lead to the issues being resolved so they may then be forced to report it to the authorities. This is sometimes called whisleblowing and some employers may decide to dismiss employees for doing this.
