This is the date from which you believe the patient may have become eligible for NHS Continuing Healthcare Funding. Whilst it could be the date of their admission to a care home, in most cases it will be the exact or approximate date upon which the patient's health deteriorated and you believe they became eligible for CHC funding. If you can't choose an exact date, please choose approximately.
This might be the date when the patient passed away, or if the patient is alive, it might be the date NHS Continuing Healthcare was awarded.
Did the patient reside in England during this period?
Has the patient's eligibility for NHS Continuing Healthcare already been assessed during this period?
